Routine Checkups & Vaccines
YEARLY CHECKUPS ARE REQUIRED. Immunizations are scheduled on an age specific basis and follow the guidelines of the American Academy of Pediatrics. In accordance with current recommendations, the vaccine schedule is as follows:

Rotateq: Rotavirus Vaccine

IPV: Inactivated (killed) Polio Vaccine

MMR: Mumps, Measles, Rubella
TDaP: Adult form of DTaP
Varivax: Chicken Pox
Influenza: The “Flu Shot” is recommended in patients 6 months of age and older and especially in patients with lung, heart disease, or other chronic illnesses. Required annually of all 6 to 59 month olds attending daycare or pre-school facilities.
Prevnar: a vaccine against Strep Pneumoniae, which can cause pneumonia / meningitis.
Menactra: Meningococcal Meningitis Vaccine
Gardasil: Human Papilloma Virus Vaccine

As you can see, there are many visits scheduled for the first two years. After that, the visits are less frequent. Nevertheless, even our older patients are seen for routine checkups on a yearly basis, not counting sick visits. This permits focus on issues involving physical and intellectual growth and development, etc. It is your chance to voice your concerns about your child's health and our chance to ask you any questions that can help us understand your needs as they pertain to your child. Patients receiving immunizations elsewhere are still required to adhere to the age appropriate routine examination schedule. This provides the parents of these children with the opportunity to ask us questions about their child, and our office the chance to update the child's medical record.
MEDICAL RECORDS, HEALTH & PHYSICAL FORMS & VACCINE RECORDS
Health forms for school, sports, or work can be submitted to the office for completion, provided the child has been examined within the last year and provided vaccinations are up to date. Otherwise, a checkup and the appropriate vaccination(s) will be required. Please allow at least one week for these forms to be completed. A fee is charged for the completion of these forms, in that they consume a considerable amount of time and resources. A copy of the child’s medical record is also subject to a processing fee.
