Referrals
If you are referred to a specialist by our office, you can make the appointment so that it is convenient for your schedule. However, it is your responsibility to make sure they accept your insurance and that you have the appropriate referral prior to the visit.
Please include the following information for all referrals:
- Patient name
- Date of birth
- Insurance and policy number
- Subscribers name and date of birth
- Home, work and cell phone number
- Specialist Name
- Specialty
- Address and phone number of the specialist
- Date of appointment
- Procedure
- Problem/diagnosis and any comments

Please give us 48 to 72 hours to obtain any referrals requiring a precertification.
Without the precertification of a referral if required by your insurance carrier, you may be responsible for the entire specialist charges at the time of service.
You will receive an email confirmation from the email address. If you do not receive an email within one full business day, please contact our office by phone.
